Sending Consultation Forms

How to send a form automatically, manually, or in-store.

Written by Kate Biel

📝 Overview

This article covers the three ways to get a form to a client; sending it automatically based on rules you set, sending or resending it manually, or filling it in together on a tablet in-store. It also walks through what the process looks like from the client's side, since they'll complete and sign every form through a secure link with no app or login required.

💡 Before You Start

  • The form needs to be Active — see Consultation Forms: Overview & Building a Form.

  • You'll need Send and view permission or higher — see Consultation Forms Permissions.

  • Each send uses one SMS credit, including resends.

🛠️ Setup

➕ How to Send Automatically

Set a rule once when you build the form, and Slick takes care of the rest — no one has to remember to send it. You choose:

  • When it goes out — as soon as an appointment's booked, a set number of hours or days before, or after the appointment (handy for aftercare or feedback forms)

  • Which services it applies to — every appointment, or just specific ones

  • How often a client needs to fill it in — every visit, or only once every few months (useful for allergy or medical history forms)

✍️ How to Send Manually, or Send Again

Click Send new form from the client's profile or the appointment to send any active form to that client, right now. If a form's already been sent and you need to send it again, use Send again from the same place.

🖥️ How to Fill In a Form In-Store

If a client's already with you and it's easier to hand them a tablet than text them a link, click Fill in store. It opens the same secure link the client would normally get by SMS.

🧑‍💻 Customer View

Clients never need an app or a login — just a secure link, sent by SMS, that opens straight to their form.

You can see this view yourself at any time without sending anything: click Preview while building a form, or go to Settings → Forms → Manage → Preview on any form.

They answer at their own pace, sign at the end, and submit. No printing, no scanning, no chasing.

Once a client completes their form, you'll get a notification in your activity feed — no need to keep checking back.

❓ Frequently Asked Questions

Find answers to common questions or additional details that may not be covered in the main instructions.

Does the form send the instant I book the appointment?

Click the arrow to view the answer

Automated forms are sent by a background process, so they may not go out the very instant a booking is made — allow a little time. If you need a form to go straight away, send it manually.

Do waitlist appointments get a consultation form?

Click the arrow to view the answer

Forms are not sent while an appointment is on the waitlist. When it is moved off the waitlist onto a real stylist, the form is sent then (if a form applies). You can also send it manually at any point.

I booked a client for tomorrow but no form went out — why?

Click the arrow to view the answer

If a booking is created inside the rule's send window (for example a "48 hours before" rule but the appointment is tomorrow), the automatic send may not go out. Send the form manually in that case.

How many SMS credits does a consultation form use?

Click the arrow to view the answer

One SMS credit per form. The message is kept short — including a shortened form link — so it stays within a single SMS.

Can I customize the text message that is sent with the form?

Click the arrow to view the answer

Not at the moment.

What happens to the form if an appointment is cancelled, deleted, or a no-show?

Click the arrow to view the answer

The form link is invalidated and no further sends go out — the same for cancelled, deleted, and no-show appointments.

If I reschedule an appointment, does the client's form link still work?

Click the arrow to view the answer

Yes. For a before-appointment form the link's expiry tracks the appointment, so moving it later or earlier updates how long the client has to complete it. A manually-sent or after-appointment link keeps its original validity.
