If your salon uses Slick to send consultation forms, you'll receive a link to your form by text message. Watch the video or find out below how to open, fill out and complete your form requested by your salon!

• You'll receive a link to your form in a text message from your salon.

• Click the link to open the form in your browser, then click Get Started.

• You'll see the questions listed one by one. Select any answers that apply to you, then proceed to the next question.

• When you reach the signature step, type in your full name and it will apply your signature automatically.

• Prefer to sign by hand? You can draw your signature out instead.

• Confirm that all of your information is complete and correct.

• Submit your form to send it to the salon.

• You'll see a confirmation page letting you know your form has been sent, along with the date and time you completed it.
