Consultation Forms: Overview and Building a Form

What Consultation Forms are, and how to build one.

Written by Kate Biel

📝 Overview

This article walks through building a form from scratch or a ready-made template, adding your own questions, and setting rules for when and how often it's sent. It also covers managing forms you've already built; reviewing, editing, duplicating, or deactivating them without starting over.

Consultation Forms are built right into Slick, so you don't need Jotform, Google Forms, or any other outside tool. You build the form once, and Slick handles getting it to the client and getting their answers back into their profile.

💡 Before You Start

  • Consultation Forms is available on the Advanced and Supercharged plans.

  • You'll need Manage permission on Consultation Forms to build or edit one — see Consultation Forms Permissions.

  • Forms only start sending once you activate them — building a form doesn't send anything to clients yet.

🛠️ Setup

Go to Settings → Forms to see all your consultation forms in one list, with active/inactive toggles for each.

➕ How to Build a Form

Learn how to build a consultation form, from choosing a template or starting from scratch to adding your questions and setting when the form is sent to clients. Watch the video below for a full walkthrough, or follow the steps underneath.

  1. Go to Settings → Forms and click New form. Pick a ready-made template (General Health, Laser Treatment, Massage Therapy, Aftercare) or start from scratch.

  2. Add your questions. Choose from short answer, long answer, yes/no, checkboxes, radio buttons, or dropdown, and switch on Required for anything you can't do without. Signature isn't a question type — it's its own toggle, switched on separately if you need clients to sign.

  3. Click Preview at any point to see exactly what the client will see.

  4. Click Save & set rules to choose when the form sends and who it applies to.

    💡Pro Tips:

    • Send form: Choose Before appointment for anything you need answers to ahead of time, like a laser patch test. Choose After appointment for aftercare or feedback forms. Choose Send manually if you'd rather a member of staff decide case by case.

    • Which services: Pick Selected services to only send a form for relevant treatments — a Laser Treatment form doesn't need to go out for a manicure booking.

    • How often per client: Choose Once per client for things that rarely change, like general health history. Choose Every appointment for forms tied to that specific visit, like a quick medication and wellbeing check before every massage.

    • Form expires after: Set a time limit if the answers can go stale — an allergy form set to expire after 6 months will prompt the client for a fresh one at their next visit past that point. Choose Never for a one-off consent form for a permanent treatment such as microblading.

  5. Review everything on one screen, then click Activate form.

✍️ Note: If you edit a form template later, forms already sent to clients keep the version they were sent — so nobody's left signing something they didn't see. New sends pick up your latest changes.

✍️ How to Manage an Existing Form

From Settings → Forms, click the pencil icon next to any form to Review, Preview, Edit, Duplicate, or Delete it, or use the toggle to switch it active or inactive without deleting it.

❓ Frequently Asked Questions

Which plans include Consultation Forms?

Click the arrow to view the answer

Available on Advanced and Supercharged. Salons on Essential or a free trial see an option to upgrade.

Do I need a separate form for every service, or can one form cover several?

Click the arrow to view the answer

You map a form to the services it applies to, and several services can point at the same form.

  • If a booking has more than one service mapped to a form, a form is sent for each.

  • If two services share the same form, it is sent once.

Example: a client who books a chemical peel and laser hair removal (each mapped to its own form) receives both, whereas Botox and filler mapped to a single "Injectables" form send just one.

We recommend grouping services into category-based forms where the questions overlap — one "Injectables" form for Botox and filler, one "Laser" form for laser treatments — so clients are not asked near-identical forms.

💡 Tip: Avoid mixing an "every appointment" form with per-service forms, or a client could receive several forms for one booking. If certain treatments need their own questions, map forms to those categories and do not also run a form set to all appointments. If most treatments share the same questions, or you are short on time, use a single form mapped to all appointments instead.

Do templates come with sending rules already set?

Click the arrow to view the answer

Yes. Built-in templates come with sensible defaults, including the sending rule (for example, an aftercare template defaults to "after appointment"). You can change the rule when you set the form up.

Can I edit a form after a client has submitted it?

Click the arrow to view the answer

No. A submitted form is stored as a record of what the client signed, so completed forms cannot be edited.

What happens if I delete a form template that is still in use?

Click the arrow to view the answer

You will not be able to send that form — the Send option is disabled and a message explains that the form's template has been deleted. To send again, use (or recreate) an active template.
